This package covers you with the major recording and reporting functions within a piece of work with children and/or their families. Some of the sections within the forms are populated with information, i.e. the definitions of parental responsibility within the referral and consent documents, plus some illustrations within the children’s consent documents. Other sections are obviously blank so you are able to populate with the information that you gather.
Please note the assessment document is a reporting template - it does not tell you what questions to ask. Should you wish to have some more guidance about this, please take a look at the Assessment Questionnaires Packs.
You will receive these seven Microsoft Word templates in a Winzip folder. Winzips are a way of transferring large amounts of data to you. They are really easy to open, often just by pressing and holding the folder icon, or right clicking the folder icon (the action depends on the brand and type of device you are using). If you are unsure, instructions are easily available on Google.
